Palliative Care at Home for Heart Failure
Living with heart failure can mean good days and hard days: shortness of breath, swelling, fatigue, and frequent trips to the doctor. In-home palliative care adds a layer of support at home, alongside the care you get from your cardiologist.
What Is the Goal of Hospice Care?
- Breathing and comfort: help managing breathlessness, especially at night or with activity
- Swelling and fluid: watching for changes and coordinating with your cardiologist on medication adjustments
- Energy and daily life: planning around fatigue so you can do more of what matters to you
- Worry and mood: support for the anxiety that often comes with a heart condition
- Planning ahead: talking through what to do if symptoms worsen, so you and your family feel ready
You Keep Your Heart Treatment
Palliative care is not hospice. You keep your cardiologist, your heart medications, and any devices or procedures that are part of your plan. We send visit notes to your doctors and coordinate with them on any changes.
Signs It May Be Time to Ask
- More frequent ER visits or hospital stays for heart failure
- Shortness of breath that limits daily activities
- Medication changes that are hard to keep track of
- A caregiver who is worried or worn out
How Visits Work
A LoveWell physician or nurse practitioner visits you at home, usually within a few days of your first call, then about once a month and more often when symptoms change. Our phones are answered 24/7.
