- LoveWell Care Guide

Who Qualifies for GUIDE?

GUIDE eligibility involves more than age or the need for a caregiver. The program is designed for people living with dementia who meet specific Medicare and living-setting requirements. A short initial conversation can help determine whether a full review makes sense.

Does the Person Have Dementia or Memory Concerns?

A diagnosed dementia condition may support eligibility review. If there are memory concerns but no diagnosis, PocketRN materials describe a process for coordinating further assessment. Memory loss by itself does not establish eligibility.

Does the Person Have Traditional Medicare?

LoveWell’s PocketRN pathway requires Original/Traditional Medicare (Parts A & B) and does not accept Medicare Advantage or other managed-care plans. If you are unsure which coverage you have, ask for help identifying your plan before assuming eligibility.

Is the Person Currently Receiving Hospice?

The current PocketRN screening checklist excludes people who are currently enrolled in hospice from GUIDE.

Where Does the Person Live?

LoveWell’s PocketRN pathway requires the person to live at home or in independent living. Other settings do not meet this LoveWell-specific residence requirement. For questions about a temporary rehabilitation stay or a particular residence, contact the screening team; do not assume eligibility from a website checklist.

Who Qualifies for the Respite Benefit?

The PocketRN flyer describes an additional respite benefit for people with moderate to severe dementia who have a family member or other unpaid caregiver. That caregiver does not need to live in the same home. Eligibility for GUIDE and eligibility for GUIDE respite are not identical.

What Happens After the Inquiry?

LoveWell can gather initial screening answers and direct the inquiry through the approved referral process. PocketRN reviews whether the person fully qualifies. Do not treat an initial checklist result as approval.

Before You Submit the Form

You can answer general screening questions without providing a Medicare identification number or sensitive medical records in the public website form. A secure process should be used if additional protected information is required.

Frequently Asked Questions

No. For LoveWell’s PocketRN pathway, the person must be over age 65, but age alone does not establish eligibility. Traditional Medicare, living at home or in independent living, dementia eligibility, and other screening requirements also matter.

Not automatically. The respite benefit described in the PocketRN flyer requires moderate to severe dementia and an unpaid caregiver, among other conditions.

A caregiver can ask about screening and next steps, but enrollment and consent requirements must be handled through the approved process.

They can request help identifying the coverage before a determination is made.

You Don't Have to Have Every Answer

You do not have to have every answer before asking for help. LoveWell Care Services can talk with you about everyday support, what your family needs, and possible next steps.

Answer a few general questions to see whether GUIDE screening may be appropriate. Completing the form does not guarantee enrollment or respite benefits.
