GUIDE Program & PocketRN
Caring for someone with dementia can raise questions that are difficult to answer alone. The GUIDE Program is a Medicare initiative designed to support eligible people living with dementia and the family members or other unpaid caregivers helping them.
LoveWell Care Services can help families explore this pathway with PocketRN. GUIDE is distinct from LoveWell’s standard hourly in-home care services.
What Support Does GUIDE Include?
The PocketRN program materials describe dementia-trained nursing support, regular check-ins, care coordination, medication review, caregiver education and coaching, help connecting with community resources, and access to a care team. Program services depend on confirmed enrollment.
Is There a Respite Care Benefit?
Qualifying families may receive up to 76 hours of in-home respite care at no cost through Medicare’s GUIDE Model. Eligibility requirements apply, and the benefit is not guaranteed for every participant.
Who May Be Eligible?
LoveWell’s PocketRN pathway requires the person to be over age 65, have Original/Traditional Medicare (Parts A & B), not Medicare Advantage or another managed-care plan, and live at home or in independent living. Dementia eligibility must also be established. The PocketRN screening materials additionally identify current hospice enrollment and certain residential settings as exclusions; confirm those source-controlled details through the current approved intake process rather than treating this page as a complete eligibility determination.
What If We Are Concerned About Memory but Do Not Have a Diagnosis?
A memory concern does not automatically mean someone has dementia. PocketRN materials describe working with existing clinicians, reviewing records, and supporting evaluation pathways when appropriate. PocketRN’s clinical team evaluates whether the person meets clinical and program requirements; enrollment remains subject to applicable CMS GUIDE eligibility and alignment requirements.
How Is GUIDE Different From Standard Home Care?
Standard LoveWell in-home care provides everyday assistance under a separate service and payment arrangement. GUIDE is a dementia-focused Medicare program with its own eligibility, coordination services, and limited respite benefit for qualifying participants.
Start With a Simple Eligibility Inquiry
Tell us whether you are caring for someone with dementia or memory concerns, whether they have Traditional Medicare, and where they live. Do not submit a Medicare number or detailed medical records through a basic website marketing form. A team member can explain the secure next step.
