Dementia Care at Home

Caring for someone with dementia can change ordinary routines. A familiar task may suddenly take longer, instructions may become confusing, or a family member may need more reminders and supervision than before. Practical help at home can support the person and the people caring for them.

Everyday Challenges Families Notice

Memory changes can affect meals, dressing, personal hygiene, appointments, and the ability to follow familiar routines. Families may also notice repeated questions, changes in sleep, confusion later in the day, or increasing caregiver fatigue.

What In-Home Dementia Support Can Look Like

Support may involve personal care, companionship, help keeping routines predictable, and relief for a family caregiver. Care should be adapted to the person’s preferences and abilities. Sudden confusion or abrupt behavior changes warrant medical attention rather than an assumption that dementia has progressed.

Support the Person, Not Just the Task

Use calm communication, allow extra time, and offer choices when practical. A consistent routine may reduce frustration. Ask the person’s healthcare team for guidance about new or concerning symptoms.

The GUIDE Program May Offer Another Layer of Support

LoveWell Care Services works with PocketRN through Medicare’s GUIDE Model to help eligible people living with dementia and their caregivers explore additional dementia-focused support. The program includes dementia-focused care coordination and caregiver education. Some qualifying families may also receive a respite benefit.

Do Memory Concerns Automatically Mean GUIDE Eligibility?

No. Memory loss alone does not confirm dementia. PocketRN can help review records, coordinate with existing clinicians, and determine next steps for an eligibility evaluation.

Frequently Asked Questions

Yes. In-home support and GUIDE enrollment are separate arrangements.

PocketRN materials describe pathways to help clarify a possible diagnosis and eligibility. A concern about memory is a reason to ask, not proof of qualification.

The current PocketRN screening checklist excludes people currently enrolled in hospice from GUIDE.
