Where We Provide Care
Where care takes place depends on the service:
- Hospice care: at home, in retirement communities, assisted living, memory care, and skilled nursing facilities
- Palliative care: in-home visits
- In-Home Care: non-medical help with daily activities at home
- GUIDE respite (Medicare dementia program): only for people who live at home or in independent living
